Endometriosis & Diet

One in ten women have endometriosis in New Zealand, yet many women are told their pain is normal.

The Ministry of Health published a paper in March 2020 on the diagnosis and management of endometriosis in New Zealand. The paper highlighted the significant impact the delayed diagnosis of endometriosis is having for those with endometriosis, leading to delays in appropriate management. The authors warned that people with endometriosis were “at risk of developing persistent pain that may impact their mental health and wellbeing”.

Endometriosis happens when the endometrial tissue (uterine lining) grows outside of the uterus, most frequently on the fallopian tubes, ovaries, intestines and other tissues. It can grow almost anywhere in the body and sheds with the normal hormonal cycle.

Endometrial tissue is a light filmy composition but its growth can be severe enough to become deeply embedded in fibrous scar tissue, restricting movement of organs in the abdominal cavity. These tissues also bleed when you have your period, regardless of where it is in the body because the endometrial tissue responds to the same hormones that cause your period.

Risk factors, Symptoms & Diagnosis

Risk factors of endometriosis include early onset of menses, heavy bleeding, IUD usage, family history of endometriosis, poor sexual relationships or sexual abuse, poor diet, or frequent genital infections.

The main symptom of endometriosis is pain; most commonly abdominal, back, vaginal and uterine pain. For some, pain occurs only with menstruation, but for many their pain is present throughout the month.

Other symptoms of endometriosis includes abnormal menstrual bleeding, infertility, gas, bloating, painful sex and urinary irregularities. Causes of endometriosis include theories such as reverse flow periods, stress, and poor immune response – but there is no consensus.  

Diagnosis of endometriosis can only be made by laparoscopic visualisation and biopsy of the pelvis. Less invasive methods, including ultrasound, may also be useful to help diagnose and manage endometriosis.

Symptoms of endometriosis are often treated with NSAIDS (non-steroidal anti-inflammatory drugs) and pain killers, which can lead to other side effects, especially gastrointestinal issues.

Scientists have more recently suggested Endometriosis is an autoimmune disease. Alongside oestrogen dominance, it now appears that the immune system may be controlling the condition. Research has found evidence that immune cells including antibodies and inflammatory cytokines are present with endometriosis. 

Leaky gut is one key factor in all autoimmune diseases. The painkillers and birth control pills commonly prescribed for endometriosis, have a negative impact on gut health, thus compounding the impact of the disease.
